Scuba diving is an exciting and rewarding activity that offers unforgettable experiences beneath the surface. The purpose of the Scuba Diving Student Medical Questionnaire is to determine whether you should seek medical clearance from a physician before participating in scuba diving or dive training activities.

Why the Medical Questionnaire Matters
